11. EHS Health Survey

Both the IACUC and EHS have responsibility for ensuring that university staff and students have a safe work environment. For EHS Health Survey, please go to the IACUC website for more information and a link to the EHS Health Survey website.

Further, information about individual workplace hazards and/or risks is gathered in the IACUC protocol review form and appendices, and in an EHS questionnaire. Faculty and staff enrollment in the EHS Health Survey program is optional for most personnel. However, all BL-3/ABL-3 lab personnel and other at-risk personnel must participate in the program.

Completion of the EHS Health Survey is mandatory for all personnel, even those with minimal animal contact. Please contact EHS and complete the online survey.
